Urinary investigations for patients with multiple myeloma (MM)3 include electrophoretic protein screening (evaluation of renal function, detection of an abnormal protein fraction) followed by immunofixation (IFE) for identification of a monoclonal component [intact immunoglobulin or monoclonal free light chains: Bence Jones proteinuria (BJP)] (1).
The algorithms cannot replace immunofixation for the determination of immunoglobulin class or urine analysis for the identification and quantification of Bence Jones proteinuria.
Although LCDD also frequently results in Bence Jones proteinuria, it is generally less prominent than in AL amyloidosis (8, 9).
The present case illustrates that on serial UPEP studies elevated B2M levels, in particular, can potentially be confused with persistent Bence Jones proteinuria because of this protein's association with renal dysfunction and electrophoretic migration pattern.
We examined 202 consecutive urine immunofixation gels that showed monoclonal reactivity with any of the 5 antisera in the Sebia Bence Jones Proteinuria assay.
The Myeloma Management Guidelines (1) recommend the Freelite test for serial monitoring of the FLCs in serum, but periodic 24-h urine collection is still required for Bence Jones proteinuria (BJP) and total urinary protein (TUP) quantification.
