A negative IGRA may be a convenient 'rule-out' test for TB if the diagnostic sensitivity of the assay is sufficiently high, for example, nearly 95%.
For individuals at high risk of infection but not at high risk of disease progression, IGRA is recommended if they have received a bacille Calmette-Guerin vaccine or are unlikely to return for TST interpretation.
There are insufficient data to recommend a preference for either a TST or an IGRA as the first-line diagnostic test in individuals 5 years or older who are likely to be infected with Mtb, who have a high risk of progression to active disease, and in whom it has been determined that diagnostic testing for LTBI infection is warranted; either test would be acceptable.
estimated sensitivity of IGRAs from studies of patients with active TB; persons in contact with patients with active TB who were categorised into gradients of exposure; and concordance of IGRA and the TST.
For these calculations, the discordance rate between the TST and IGRA was arbitrarily set at 30%.
