The pain of shoulder syndrome along the upper scapular border has been attributed to the overstretch and fatigue of the levator scapula and rhomboid muscles that occur when denervation of the trapezius leads to loss of scapular support and stability.
Further assessment revealed that the manual muscle test strength for the rhomboid muscles was 4/5.
The nerve then crosses the posterior triangle of the neck on the anterior surface of the levator scapulae and rhomboid muscles.
