Either of a pair of paranasal sinuses located centrally between and behind the eyes, below the ethymoid sinus.

Secretions from the sphenoid sinus and posterior ethmoid sinus generally proceed posterior and superior to the eustachian tube orifice.
Curvilinear planar reformation with vessel extraction showed a persistent narrowing of the cavernous segment of the internal carotid artery in proximity of the posterior sphenoid sinus wall fracture, with intimal flap at the inferior aspect consistent with dissection.
Contiguous noncontrast coronal 2-mm slices were obtained extending from the anterior frontal sinuses to 2 cm posterior to the posterior wall of the frontal sinuses; in addition, contiguous 5-mm slices to the posterior wall of the sphenoid sinus were performed.
