The thigh adductor muscles include gracilis, adductor longus, adductor brevis, adductor magnus and pectineus muscle. The first four muscles are innervated by obturator nerve, while pectineus muscleis innervated by femoral nerve in 90 % cases (Woodburne, 1960; Yoshida et al., 2017).
After prepped, the leg was abducted to approach the pubic segment with dissection and preservation of the insertion of the pectineus muscle, followed by approximately one centimeter pubic ostectomy near the medial aspect of the acetabulum.
The cyst was located between the pectineus muscle and sartorius muscle, accompanied with anterior bulging to subcutaneous fat.
With high-frequency linear probe (L9-3), the mass with US features of edematous intestinal wall was more clearly depicted, and we found that it was deep to the pectineus muscle and common femoral vessels [Figure 1]b and [Figure 1]c.
Branches to the pectineus muscle, sartorius muscle, and sensory nerves (saphenous and medial cutaneous) branched first and 'medially in 20 out of 25 specimens.
Pectineus muscle atrophy may contribute to femoral hernia development, creating a potential space for the hernia to form.
There was herniation of a bowel loop of length 3.5 cm through left obturator foramen extending inferiorly between pectineus muscle anteriorly and obturator externus muscle posteriorly, suggestive of obturator hernia (Figures 3, 4(a), and 4(b)).
Although, abdominal ultrasound was found to be normal, computed tomography of the abdominal cavity showed bilateral psoas abscess that extended to pectineus muscle and also involved the iliopsoas muscle on the right.
The pectineus muscle was divided transversely 1 cm below its origin and turned toward the femur.
The pectineus muscle generates low forces easily counteracted by other thigh muscles, and the saphenous and medial cutaneous sensory nerves are unessential to standing.
A multiloculated hydatid cyst between the iliopsoas and pectineus muscle had been diagnosed using an ultrasonography and magnetic resonance imaging (MRI).
