SNOMED International is a not-for-profit organization that owns and develops
SNOMED CT, theworld's most comprehensive healthcare terminology product.
The group is currently working with data elements related to CLABSI (peripheral and central lines), with a goal of coding them to LOINC and
SNOMED CT in 2017.
Once documentation screens are created that have
SNOMED CT coded summative assessment, process, and outcome indicators for all specified CCTM activities, then documentation data sets can be mined to see which types of providers (identified through coding of license type) are doing which assessments and processes, and the outcomes achieved.
The framework links
SNOMED CT codes to the MTM services that pharmacists perform to document and communicate this information with other health care providers and payers through electronic health records and other standardized systems.
The eCC format is ideal for recording most biomarker results and allows interoperability and transmission of results without reentry of data for separate sites and mapping to common terminologies including Human Genome Variation Society (HGVS) nomenclature, (26-28)
SNOMED CT, and LOINC.

In early 2007, in New South Wales (NSW), Australia, the Systematized Nomenclature of Medicine--Clinical Terms (
SNOMED CT) was introduced for diagnosis coding, as part of the roll out of a new information system for EDs, called FirstNet.
The Systematized Nomenclature of Medicine--Clinical Terms (
SNOMED CT) is the terminology required for the capture of problem lists under Stage 2, causing some concern among clinicians and executives who have not previously considered how SNOMED will play a role in their IT strategies.
Tasks may include translation assistance to international vocabulary standards, such as LOINC or
SNOMED CT.
The EMR links the physician's notes to the
SNOMED CT concept (H/O: thrombosis) or an ICD-9-CM concept (personal history of venous thrombosis and embolism).
