Hyaline lamellae with focal scolex representing the membraneous mucoid material on gross description (ectocyst,
endocyst and pericyst) (HE, x200); C.
The hydatid cyst stages are a cyst with a visible cyst wall (stage 1), multiseptated, multivesicular cysts (stage 2), a cyst with a detached floating
endocyst membrane (stage 3a), a unilocular cyst with solid or mucinous matrix and internal daughter cysts (stage 3b), a heterogeneous solid cyst with a degenerative internal canalicular structure (stage 4), and a degenerated cyst with heavily calcified wall (stage 5) [17].
Cyst was removed in toto along with
endocyst and ectocyst (Figure 3).
Incision on the pelvic peritoneum and the cyst wall revealed a white
endocyst. Endocystectomy with partial pericystectomy was done without any intraperitoneal spillage and the margins of the pelvic peritoneum and remnant ectocyst sutured.
Total peric./liver resec., 27 (8%)
Endocyst and capitonnage, 61 (18%) External drainage, 10 (3%)
Endocyst and partial/subtot.
(5) Structurally the cysts consist of a tough outer pericyst that protects a delicate inner
endocyst from which brood capsules and daughter cysts develop.
There was a significant collection of eosinophils, plasma cells, macrophages and few neutrophils around the
endocyst.
The communicating type consists of a tear of the
endocyst with escape of contents via biliary radicles or bronchioles.
Within the pericyst lies the external layer of the cyst itself (ectocyst) and the inner germinative membrane or the
endocyst. The cyst is filled with fluid and may contain several daughter cysts.
The outer layer is the pericyst, which consists of compressed liver; inside this is the
endocyst, a germinal layer; and inside this is the ectocyst, a thin membrane.
