As there are well-preserved people with elevated renal parameters and in those without any obvious etiology, the question arising is whether it is worthwhile to speculate if this entity is some form of
interstitial nephritis caused by some environmental contaminant, food additives, or habits that per se are contributory or causative factors and accelerated by other coincidental diseases or comorbidities.
The most common NKD in the current study population was Chronic Glomerulonephritis followed by Chronic
Interstitial Nephritis. Even in other studies around the globe, the glomerular disease was the common NKD.
Renal biopsy was performed, revealing an eosinophilic infiltrate suggestive of acute
interstitial nephritis (allergic type), and seguelae of vasculitis, with no signs of acute activity (Figure 3, 4).
Both granulomatous
interstitial nephritis and renal dysfunction from extrarenal sarcoidosis are also responsive to corticosteroids.
Histological characterization of tubular
interstitial nephritis includes inflammatory changes in the tubulointerstitial compartment, such as leukocyte infiltration, atrophy, accumulation of extracellular matrix proteins, tubular dilation, and interstitial edema.
Histopathological evaluation of the right kidney revealed chronic
interstitial nephritis. There was a difuse mononuclear cell infiltration in the cortex.
Acute
interstitial nephritis (AIN) is a characteristic inflammatory infiltrate in the renal tubule-interstitium that is a leading cause of acute renal failure.
Other renal inflammatory conditions including renal parenchymal malakoplakia and megalocytic
interstitial nephritis need to be differentiated from XGP based on their histologic characteristics [25].
Another similar observation of a case of MM presenting as acute
interstitial nephritis and RA-like polyarthritis was reported by Ardalan and Shoja in 2007.
Moreover, no guidelines or recommendations for screening have been made to date in patients with
interstitial nephritis [2].
These cases were diagnosed as either acute tubular necrosis (ATN) or acute
interstitial nephritis (AIN)2.
The spectrum of renal disease includes
interstitial nephritis, which can manifest as distal RTA, proximal RTA, tubular proteinuria, nephrogenic diabetes insipidus, glomerular disease, or renal failure (1,8).
