in December 1987, a chest radiograph obtained from one of his brothers indicated a small patchy lesion in the right upper lobe; this brother had a history of tuberculous pleural effusion in 1955 for which he was treated with isoniazid (INH), para-aminosalicylic acid, and streptomycin for an unknown duration.
DST was conducted for 10 drugs: rifampin (1.0 [micro]g/mL); isoniazid (0.2, 1.0, and 5.0 [micro]g/mL); ethambutol (5.0 [micro]g/mL); ofloxacin (2.0 [micro]g/mL); ciprofloxacin (2.0 [micro]g/mL); kanamycin (5.0 [micro]g/mL); capreomycin (10.0 [micro]g/mL); amikacin (4.0 [micro]g/mL); para-aminosalicylic acid (2.0 [micro]g/mL); and ethionamide (10.0 [micro]g/mL).
On the basis of the DST results, the treatment was changed to ethambutol, pyrazinamide, amikacin, moxifloxacin, para-aminosalicylic acid, and linezolid.
