The diagnosis of hypocalcaemia-induced coronary artery vasospasm is difficult to make in the acute setting, especially in a patient with multiple risk factors such as CKD and HIV infection.
For patients with aneurysm rupture, fourteen days after attack is the peak period of aneurysm rupture; vasospasm is of high risks three days after attack; the incidence of vasospasm is the highest seven days after attack.19 Vasospasm can affect catheterization and determination on the size of aneurysma.
In regard to neurological risks to mobilisation (refer to Table 2), the majority of physiotherapists reported that they would prefer patients to remain in bed when vasospasm is present (n=38/51, 75%), DCI (n=36/50, 72%) or there has been a recent further bleed (n=41/52, 79%).

It has been suggested that coronary artery vasospasm may occur from cannabis use, which may precipitate thrombosis; however, this is not well defined.
Aneurysmal subarachnoid hemorrhage (aSAH) is often associated with poor functional and cognitive outcomes, (1) which are attributed to the initial hemorrhage, as well as secondary complications such as cerebral vasospasm, delayed cerebral ischemia, or myocardial injury.
(3,4) Therefore, adrenaline should be used cautiously due to the theoretical risk of worsening coronary vasospasm and myocardial ischemia through the alpha receptor effect.
ACh provocation was not performed to test for vasospasm. We postulate seizure-induced hypercatecholaminergic-mediated coronary vasospasm at the mid-RCA as the cause of the STEMI.
In rare instances, patients with hyperthyroidism can experience angina suggestive of myocardial ischemia either due to known or suspected underlying ischemic heart disease or secondary to coronary artery vasospasm (CAS) [2].
Ergotamine mimics the neurotransmitters norepinephrine, serotonin, and dopamine and can result in smaller and bigger vessels vasospasm and vasoconstriction [3].
Vasospasm is a devastating event, complicating as high as 50% of SAH with increased morbidity and mortality [1].
Vasodilators are the mainstay of treatment in usual vasospasm, but in patients with CSS induced vasospasm, vasodilators have little role in the treatment [8].
