In syphilis, tuberculosis, and cowpox, a chancre.
In dermatology, the earliest clinically recognizable manifestation of cutaneous disease, such as a macule, papule, vesicle, pustule, or wheal.

When the primary lesion is in the central tegmental tract, olivary degeneration is ipsilateral, as occurred in our patient.
The primary lesions are black dots, papules, nodules and burrowing excoriations.
In our case primary lesion and mandibular metastasis were detected simultaneously, and mandibular metastasis probably grew up from the periapical region, resorbed the alveolar bone and then infiltrated to the gingival soft tissue around right premolar teeth.
