Computed tomography demonstrated a soft-tissue mass in the middle ear space and a bony expansion of the fallopian canal from the geniculate ganglion to the stylomastoid foramen.
The mastoidectomy is performed first, and it involves a standard postauricular facial nerve decompression that extends from the stylomastoid foramen to the beginning of the geniculate ganglion.
It then curves (the pyramidal bend) to course vertically through the mastoid bone and exits at the stylomastoid foramen (figure 2).
