Use a locking pliers at the base of the old fill valve (inside the tank) to keep it from spinning as you remove the locknut (on the outside of the tank) with an adjustable pliers.
Just remove the old head using a long-handled adjustable pliers. Then apply thread joint sealant or Teflon thread-sealing tape to the threads (wrap the tape on clockwise - the same direction the shower head screws on).
