In a stepwise multivariate Cox model, four significant variables were retained: Q-wave, VT, IVET+, and SDNN<100 ms.
During analysis of the complications in patients with MI without Q-wave MI it was discovered that re-infarction was recorded in 10.
Significance of Q-wave regression after anterior wall acute myocardial infarction.
Eight percent of the patients showed evidence of Q-wave UMI, and 27 percent had evidence of non-Q-wave UMI.
The problem is, not all UMIs result in Q-waves on the electrocardiogram.
Patients were considered to have either an ST-elevation MI (ST elevation or Q-wave with an increased cTnI) or a NSTEMI (no ST elevation or Q-wave but with an increased cTnI) on the basis of both serial ECG and cTnI determinations over 12-24 h after presentation for both the initial or reinfarction events.
The main outcomes evaluated were death, cardiac death (defined as sudden, unexpected death or death related to myocardial infarction), nonfatal myocardial infarction (new Q-wave in 2 or more electrocardiogram leads), the need for repeat revascularization for proven ischemia demonstrated by either follow-up cardiac events or a positive noninvasive stress test with significant angiographic stenosis of at least 50%, or a composite of these outcomes.
MACCE is defined as Composite of Major Adverse Cardiac and Cerebrovascular Events, including Death/Stroke or TIA/MI/Repeat Revascularization in the PROTECT II study, using 8xULN for biomarkers or Q-wave for Peri-procedural MI (Stone et al Circulation 2001;104:642-647) and 2xULN for Spontaneous MI (PROTECT II definition).
Objective: Significant Q-wave is sometimes invisible in the patients with triple vessel disease (TVD) even though TVD is a serious coronary heart disease.
MIs included all new ST-segment elevations, Q-wave events, left bundle branch blocks, and elevations in serum levels of creatine kinase that exceeded three times the upper limit of normal.
