On her electrocardiographic examination ventricular tachycardia had been detected and it was ceased with antiarrhythmic drugs but on echocardiographic examination huge
mural thrombus on left ventricle has been detected.
Second, they can delineate and localize the DPA aneurysm with accuracy and they can identify the presence of
mural thrombus. Finally, they are of great help in the assessment of the adequacy of foot perfusion perioperatively, and therefore, help in the decision of whether to ligate or to reconstruct the aneurysm.
The excised segment was dilated and contained
mural thrombus (Figure 2; T indicates thrombus).
Additional investigations included a 24 hour ECG which was normal and a Trans-thoracic Echocardiogram that showed overall good left ventricular systolic function and non-dilated cardiac chambers with no evidence of
mural thrombus. All valves appeared structurally normal.

Patients with previous history of myocardial infarction, valvular heart disease, dilated Cardiomyopathy and
mural thrombus were excluded.
Atrial fibrillation was observed in 53 patients and
mural thrombus in 19 patients.
The focal dilatation within the proximal right coronary artery contained a small amount of nonocclusive
mural thrombus, as well as a thin rim of calcification (Figure 4).

The definitive imaging modality in confirming graft dilatation is coronary angiography, though
mural thrombus may at times obscure the true dimensions of the aneurysm.
The distal neck was also markedly conical with a diameter of 42 mm at the coeliac axis, with extensive
mural thrombus. These features rendered a successful endovascular repair unlikely, and therefore endovascular interventional was discounted.
