The classical features of LP include abdominal pain (lead colic), anemia with basophilic stippling
of red cells, blue-black gum deposits (Burton line), and lead line on joint radiography [11-13].
Another feature was basophilic stippling
, which were prominent in HbS disease and occasional in HbS trait.
The abnormalities seen on the smears examined revealed the presence of nucleated red cells, basophilic stipplings
, Target cells, Stomatocytes and Tear drop cells.
Presence of anemia, leukopenia, thrombocytopenia, or eosinophilia on complete blood count, basophilic stippling
on the peripheral smear, or elevated liver transaminases is consistent with arsenic exposure but is not specific.
Lab work reveals a low hematocrit and hemoglobin, elevated reticulocyte count, and basophilic stippling
. You suspect lead poisoning.
Her blood film showed basophilic stippling
that was morphologically distinct from Pappenheimer bodies or Howell Jolly bodies.
In opium users described by van't Klooster et al., the basophilic stippling
of erythrocytes, possibly a consequence of lead-induced ribonuclease inhibition, plus the gingival lead line, enhanced the clinical suspicion of plumbism; however, these findings are neither clinically sensitive nor specific.
The peripheral smear examination showed anisopoikilocytosis with macrocytes, microcytes, schistocytes, target cells, basophilic stippling
, and hypochromia of RBCs.
Complete hemogram was done for all cases by electronic cell counter (MSQ haematology cell counter, Cergy Pontois, France), supported by manual hemocytometry and peripheral blood examination (Leishmain stain) RBC morphology regarding presence of hypochromia, microcytosis, anisocytosis, poikilocytosis, sickle cells, target cells, polychromasia, basophilic stippling
and other inclusion bodies were also assessed.
