In most women, this incision cuts a portion of the bulbospongiosus muscle and, occasionally, reaches the ischioanal fossa.
The structure of the tunica muscularis varies along the length of the duct, ranging between three layers near the bladder and a striated urethral muscle in more distal sections, where it becomes continuous with the bulbospongiosus muscle. The outer covering of the urethra is the tunica adventitia.
We believe that the damage to the perineal body, superficial transverse perineal muscles, and bulbospongiosus muscles, which provide contribution to anal continence, is ignored, especially in direct surgical repair.
