The substance, extracted from animal tissues, is used clinically to delay blood clottingblood clotting,
process by which the blood coagulates to form solid masses, or clots. In minor injuries, small oval bodies called platelets, or thrombocytes, tend to collect and form plugs in blood vessel openings.

Heparinoids are proteoglycans that have been developed as anticoagulants.
Low-molecular-weight heparins, heparinoids, ancrod, argatroban, and hirudin have been used in a significant number of patients.[4-6,9,75,98-100] A small clinical trial was conducted with ancrod, a viper-derived venom with anticoagulant action that showed efficacy.[101] Other reports describe the use of plasmapheresis to remove immune complexes,[102] or high doses of immunoglobulin alone[103] or associated with LMWH and a heparinoid.[104] Also indicated for use in patients with HIT II are thrombolytic agents; the insertion of filters in the inferior vein cava; and, in the case of arterial thrombosis, with limb ischemia, embolectomy, or thrombectomy.[4-6,92]
We have been recently described that Hf-SP are heparinoids. The anticoagulant activity determined by APTT test for crude fractions Hf-SPb [Hf-SP.sub.2] and [Hf-SP.sub.3] about 37, 68 and 36 IU [mg.sup.-1], respectively.
A hypocoagulation state may occur in severe systemic inflammatory response syndrome or in sepsis, which may be caused in part by endogenous heparinoids (1).
Pentosan is a low-molecular-weight heparinoid compound that has anticoagulant and fibrinolytic effects.
In patients undergoing minor surgery or procedures who receive bridging with treatment-dose LMWH, the heparinoid may be resumed 24 hours after surgery if there is adequate hemostasis.
Danaparoid, a low-molecular-weight heparinoid with low placental permeability, is considered by some to be the drug of choice for heparin allergy in pregnancy, (12,17-19) but is currently not available in South Africa and was therefore not an option in this patient.

If continued anticoagulation is required, alternative therapies are available such as low molecular weight heparin (LMWH), enoxaparin sodium/Lovenox[R], or the heparinoid, danaparoid sodium/Orgaran[R].
Heparinoid metabolism appears to significantly alter in pregnancy.
