Differential diagnosis is made with marantic endocarditis, associated with neoplastic processes, for which advocates the fever and the antral gastric cancer, but where the echocardiographic image is typical with numerous tiny vegetations, with frequent embolic complications and severe prognostic (unlike our case); endocarditis in collagenoses, specific for the female gender, that also associates fever, tiredness, weight loss, but with no systemic complications and no biological features of colagenosis in our case.
(7,8) It is possible to find cases of non-bacterial thrombotic endocarditis (marantic endocarditis), and this is present in 3-5% of AIDS patients, mainly in those with wasting syndrome.
Marantic endocarditis shows lesions located over the cusps and the patient usually has Hypercoagulable state like APLA syndrome.
