A #2 FiberWire (Arthrex, Naples, FL) is used in a running-locking fashion to imbricate the elongated medial and lateral retinacula and oversew the tendon repair (Figure 2(b)).
All these fibres are attached to the joint capsule, forming the medial and lateral patellar retinacula. The posterior surface of the patella ligament is separated from the synovial membrane of the joint by a large infrapatellar pad of fat, and from the tibia by a bursa--the deep infrapatella bursa.

(Note that these muscles recently have been renamed "fibularis longus" and "fibularis brevis." (38,39) Their accompanying tendons, retinacula, nerves and vessels have also been similarly renamed.
(3) This region of avascularity also corresponded to the location of superior and inferior retinacula, which serve as pulleys for the ATT mechanism.
(2) The quadriceps tendon, medial and lateral retinacula, iliotibial band (ITB), vastus medialis and lateralis, and the insertion of the patellar tendon on the anterior tibial tubercle all play a role in proper tracking of the patellofemoral joint; an imbalance in any of these forces leads to abnormal patellar tracking over the femoral condyles, and pain ensues.
Full extension may nevertheless be possible if medial and lateral retinacula are intact.
